TL;DR¶
This paper proposes Reasoning Inception (ReIn), a test-time intervention method that requires no modification to model parameters or system prompts. An external inception module detects conversational errors and injects recovery plans into the task agent's reasoning chain, significantly improving task completion rates across diverse error scenarios while generalizing to unseen error types.
Background & Motivation¶
LLM-driven conversational agents perform well in tool-integrated tasks, yet face unpredictable user-induced errors in real-world deployment:
Underestimated user-side errors: Users frequently issue ambiguous requests (unclear references, ambiguous interpretations) or requests exceeding system capabilities (unsupported operations, parameters, or domains).
Error recovery vs. error prevention: Prior work focuses primarily on error prevention (clarification, fallback) rather than diagnosis and recovery after errors occur.
Practical constraints: In deployed systems, model parameters and system prompts of task agents are typically calibrated and fixed; modifying them is costly and may introduce side effects.
The core challenge is: how can an agent diagnose problems and execute recovery when encountering user errors without modifying model parameters or system prompts?
Method¶
Overall Architecture¶
At the start of each dialogue turn, ReIn employs an external inception module to detect potential errors, generates a reasoning block containing a recovery plan, and injects it into the task agent's internal reasoning context.

Key Designs: The ReIn Mechanism¶
Inception module \(F\): Given the surface-level dialogue context \(\{\mathcal{C}_t, u_t\}\), tool list \(\mathcal{L}\), and error-recovery mapping \(\Phi: \mathcal{E} \to \mathcal{T}\), the module outputs:
- No: No known error detected; dialogue proceeds normally.
- (Yes, ρ_t): An error is detected, and \(\rho_t \in \mathcal{T}\) is the recovery plan.
Reasoning injection: $\(r_t = \begin{cases} \varnothing & F(\{\mathcal{C}_t, u_t\}, \mathcal{L}, \Phi, \mathcal{S}') = \text{No} \\ \texttt{think}[\rho_t] & \text{otherwise} \end{cases}\)$
\(r_t\) is wrapped in think tags and injected into the agent's internal context: \(\hat{\mathcal{C}}_t = \tilde{\mathcal{C}}_t \cup \{r_t\}\), after which action sampling proceeds on the augmented context.
Error Taxonomy and Recovery Plans¶

Key design: Contradiction and Domain errors are marked as UNSEEN and excluded from the inception module's prompt, serving as a test of generalization.
Relationship to the Instruction Hierarchy¶
Following the instruction hierarchy of Wallace et al.: System Message >> User Message >> Model Outputs >> Tool Outputs. ReIn belongs to Tool Outputs (lowest priority), yet experiments show that when paired with recovery tools defined via JSON schema, ReIn can effectively influence agent behavior. Without corresponding tool definitions (relying solely on textual instructions), the agent follows the system prompt and ignores ReIn (0% success rate), empirically validating the instruction hierarchy.
Loss & Training¶
ReIn is a test-time intervention method and involves no training or loss functions. The inception module leverages existing LLMs without additional training.

Experiments are conducted on a τ-Bench adaptation comprising 98 dialogue sessions and 588 context instances (392 seen, 196 unseen).
Sonnet 3.7 as task agent, effect of different inception modules (retail domain Pass@1):

ReIn consistently improves task completion across all inception modules. Without ReIn, Pass@1 for ambiguous scenarios approaches 0%.

ReIn surpasses both prompt-modifying baselines without requiring any modification to the system prompt.
Ablation Study / Generalization Analysis¶
Generalization to unseen error types: ReIn effectively identifies and recovers from Contradiction and Domain errors (unseen types), in some cases even matching or exceeding performance on seen error types.
Dynamic vs. fixed triggering: Allowing ReIn to activate dynamically each turn (rather than only at predefined error turns) further improves task completion in most scenarios.
Limitations of the 3B model: The smallest inception module exhibits a substantially lower activation rate than larger models (Sonnet 3.7 approaches 100%; the 3B model is notably lower), attributable to limited long-context comprehension in smaller models.
Key Findings¶
- Empirical validation of the instruction hierarchy: ReIn belongs to Tool Outputs (lowest priority), but pairing it with JSON schema tool definitions can effectively "bypass" the hierarchy; success rate is 0% without tool definitions.
- Error type differences: Baseline Pass@1 for unsupported scenarios (~20%) is higher than for ambiguous scenarios (~0%), as the system prompt already includes brief guidance for human handoff.
- Strategic decision-making by ReIn: Case studies show that ReIn can proactively escalate to a human agent when users persistently insist on erroneous information, demonstrating flexibility beyond predefined scenarios.
Highlights & Insights¶
- Effective solution under extreme constraints: Reasoning injection alone substantially improves recovery under the strong constraint of no parameter or prompt modification.
- In-depth analysis of the instruction hierarchy: This work provides the first empirical study of ReIn's interaction with the instruction hierarchy, identifying tool definitions as the key mediating factor.

Limitations & Future Work¶
- The error taxonomy is relatively simplified (only 6 types); real-world deployments involve far greater error diversity.
- Evaluation is based on an adapted τ-Bench with limited dialogue turns and scenario variety.
- The inception module introduces additional computational overhead, requiring one extra LLM call per turn.
- Experiments are conducted only on Claude-series models; applicability to other agent frameworks remains unknown.
- Incorrect recovery plans generated by ReIn are directly counted as failures, without any fault-tolerance mechanism.
Related Work & Insights¶
- Relationship to RAG: RAG addresses knowledge gaps; ReIn addresses conversational deviation. The two are complementary.
- Relationship to prompt injection research: ReIn is essentially a "safe prompt injection" that requires service-provider-authorized tool definitions, conforming to the instruction hierarchy.
- Implications for real-world deployment: ReIn offers a lightweight error recovery augmentation approach for scenarios where agents are already deployed and cannot be easily modified.
- Comparison with Self-Refine: Self-Refine requires prompt modification and yields lower performance; ReIn achieves greater efficiency through reasoning injection.
